- Make sure to complete all new patient forms within the patient portal before your appointment.
- Please use the insurance benefits verification form to understand your coverage before your first visit.
- If our office is out-of-network with your insurance, the payment is due at the time of service.
- Review our Privacy Policy.
- Please bring in a list of your medications and any recent laboratory results and diagnostic imaging.

Dr. Naydis, Dr. Apfelbaum, and Dr. Sims-Day are a preferred (in-network) provider with:
- Regence
- Premera
- Lifewise
- First Choice
- Kaiser Permanente (select plans)
- Aetna (Dr. Naydis and Dr. Sims-Day only)
- They also work with with PIP (motor vehicle accident insurance), and L&I claims (worker's compensation)
- Dr. Apfelbaum and Dr. Naydis are certified medical examiners for the DOT exams
